$75.00 ENTRY FEE PER TEAM (TEAMS MUST BE 3 MINIMUM 6 MAXIMUM)

SINCE THIS IS A NEW EVENT, WE WILL BE PRE REGISTERING FOR THIS EVENT

MUST BE REGISTERED BY JULY 10, 2010

ALL REGISTERED TEAMS WILL RECEIVE AN EVENT T SHIRT FOR EACH PERSON ON YOUR TEAM.

LATE REGISTRATIONS WILL NOT RECEIVE T SHIRTS THE DAY OF THE EVENT - BUT CAN PICK THEM UP AFTER AUGUST 1, 2010

PRE REGISTRATION WILL CONSIST OF THE FOLLOWING BREAKDOWNS:

ELEMENTARY AND JR. HIGH DIVISION - 15 AND UNDER

HIGH SCHOOL AND OLDER DIVISION - 16 - 39

OLD FART DIVISION - 40 AND OLDER

FAMILY DIVISION - ANY AGE COMBINATIONS

co-ed - all ages - 3 on 3 half court basketball tournament REGISTRATION FORM 2010

Minimum of 3 players per team; Maximum of 6 players per team.

Everybody competing in their division must be under the maximum age allowed in the division (NO exceptions).

Age as of day of tournament - July 24, 2010.

By signing my name below I agree to this waiver agreement. Age 17 and under must also have parent/guardian part of the form signed

I fully agree that I am physically fit and able to participate in the Hoops in the Hills Elk River 3 on 3 Tournament.

I fully understand that there is a risk of physical injury and am willing to accept that possibility.

I fully agree that it is my responsibility to understand and obey all rules and laws to ensure my safety.

I fully understand that any organizations involved in the planning, facilities and coordination of the tournament as well as any of its sponsors and any associated individuals aren't responsible for any loss, injury, or death related to participation or attendance at the 3 on 3 Tournament.
I fully agree to know and abide by all NCAA rules.
Four different 3 on 3 Divisions:

Elem and Jr. High 15 and under

High School and older 16-39

Old Farts 40 and older

Family Teams - any age combinations

Parent/Guardian Form - Any player 17 and under must have parent/guardian sign the form following registration please make as many copies as you need for each player 17 and under.

We are looking for local sponsors for our First Annual 3 on 3 Half Court Basketball Tournament

***Deadline for Banner Info and Artwork is next Friday, May 28, 2010******

Pepsi will be donating 50 event flyers and two 2’x8’ Sponsor Banners which will be displayed on the top of the fence at this event, they are also donating Gatorade for the Participants

RAC is having Event/Sponsor T-Shirts made for the Participants and for purchase by the spectators

Prizes this year will be Ribbons 1^st through 3^rd Place (68 Ribbons)

And a % of Entry Fee will go towards the !st Place Cash Prize award in each division

If enough donations come in RAC will be providing programs with the pre-registered teams and brackets for spectators to purchase with all of our sponsors listed as well
